Curis Partners and Curis Medical PLLC (“Curis”) are in-home health service providers that deploy technology and a skilled clinical workforce into patients’ homes to avoid unnecessary utilization and improve health outcomes. Curis Partners has over 60 advanced care clinicians (MD, PA and NP) conduct health-risk assessments, provide primary care, to patients that have difficulty leaving the home, and transitional care, to patients recently discharged from the hospital. Their providers currently cover the 5 boroughs of NYC, Nassau and Westchester counties, using proprietary software to document their home visits. This tablet application, together with their Home Visit Management System enables Curis to efficiently schedule, document, monitor, code, bill and report the patient’s condition and all of our activity. Fundamental to the assessment process, Curis uses a risk-scoring methodology considering environmental, socio-economic, and clinical factors discovered during the initial home visit to identify patients that are at imminent risk of an ED visit or hospitalization. These findings are summarized and shared with all members of the care team (PCP, Specialists, Care Managers) within 24 hours of the visit. Immediate access to this data enables the care team to collaborate and create the appropriate intervention to mitigate an exacerbation of the condition. Curis managed care plan and risk contracted clients receive multiple benefits from their services: increased capitation revenue through accurate and complete diagnosis coding, improved quality (STAR) measures , HEDIS capture, closed gaps in care and reduced medical expense by avoiding unnecessary hospitalizations and ED visits.
